  • G-845, Immigration Verification Request. Please note: this document must be accompanied by a statement with the ORIGINAL SIGNATURE and DATE of the applicant that states that the applicant authorizes the U.S. Citizenship and Immigration Services to release information regarding his immigration status to [name of lender] because he/she is applying for a U.S. Small Business Administration loan.
