SBA 1919, Borrower Information Form – Revised 2018. This is to be completed by all owners of 20% or more of the business, all guarantors, officers, managing members (LLC) and any person hired by the business to manage the day-to-day operations of the business.
SBA 912, Statement of Personal History. This form is no longer required on all SBA loans, only in situations where the principal is not a U.S. Citizen, has a criminal history, or it is otherwise required by the SBA.

G-845, Immigration Verification Request.Please note: this document must be accompanied by a statement with the ORIGINAL SIGNATURE and DATE of the applicant that states that the applicant authorizes the U.S. Citizenship and Immigration Services to release information regarding his immigration status to [name of lender] because he/she is applying for a U.S. Small Business Administration loan.
